This appointment is strictly for existing patients ONLY who has already done an initial evaluation with your physical therapist. Schedule a physical therapy treatment session for continued care with your therapist. Please Note: If you have not seen your physical therapist greater than 2 months for the same injury/condition, please email info@cyncpt.com to schedule a Re-Evaluation with your physical therapist.
