Physical Therapy Initial Evaluation

Service Description

This appointment is for new patients or patients who has a new injury. An initial evaluation is a full ONE hour appointment with your physical therapist. During this time your physical therapist will take your history in relation to your injury, do a full body assessment, diagnose, and educate you on a plan of care. Please Note: If you are a new patient you must get your insurance verified prior to booking. To verify insurance, please click on "check coverage" under the INSURANCE link.
